Rural communities are at a pivotal moment in America’s healthcare landscape. As states compete for Rural Health Transformation funding, a clear pattern has emerged: leaders are no longer focused on incremental fixes but on redesigning the rural care ecosystem from the ground up. Across applications, five priority areas consistently rise to the top—telehealth expansion, workforce stabilization, integrated behavioral health, strengthened primary care access, and the data infrastructure required for value-based care. This week’s Friday Five highlights these key priorities and what they mean for the future of rural healthcare.
Together, these priorities reveal a shared national strategy: equip rural providers with the tools, technology, and partnerships needed to deliver high-quality care in environments defined by distance, workforce shortages, and shifting demographics. The result is a blueprint for rural health that is more connected, more coordinated, and more resilient than ever before.
Telehealth & Remote Monitoring
States overwhelmingly emphasized expanding telehealth capacity, broadband connectivity, and remote patient monitoring to reach residents in geographically isolated areas. This includes virtual primary care, behavioral health, chronic disease management, and remote diagnostics.
Workforce Expansion & Retention
Many states focused on strengthening the rural workforce pipeline—recruiting clinicians, supporting rural residencies, expanding community health workers, and enabling providers to practice at the top of their license.
Integrated Behavioral Health & Social Services
Applications frequently included models that integrate behavioral health with primary care and social supports, aiming to address mental health needs holistically and reduce fragmentation.
Primary Care Access & Care Coordination
States prioritized improving primary care infrastructure, reducing barriers to access, and strengthening coordination between primary care, specialty care, and behavioral health.
Value‑Based Care & Data Infrastructure
A major theme across applications was preparing rural providers for value‑based payment models by investing in data systems, analytics, population health tools, and reporting capabilities.
