Health Centers Delivered High Quality Care to a Record Number of Patients

August 25, 2025 Posted by Industry News HHS Agencies News

HRSA-Funded Health Centers Served Over 32 Million Patients, an Increase of 1.1 Million Patients

The Health Resources and Services Administration (HRSA) released new data showing HRSA-funded health centers served over 32.4 million patients in 2024 — the most patients ever served in the Health Center Program’s 60-year history.

“Our nation’s health centers are key to addressing chronic diseases and improving health outcomes in communities across the country,” said HRSA Administrator Tom Engels. “As we celebrate National Health Center Week, I want to thank our health center heroes for their tireless commitment to providing exemplary care to their communities, this week we recognize their tremendous accomplishments.”

HRSA-funded health centers provide comprehensive primary care at more than 16,000 service sites nationwide to 1 in 8 children, 1 in 5 rural residents, and 1 in 15 adults 65 and older. These health centers are at the forefront of the efforts to Make America Healthy Again and continue to be leaders in quality of care. Quality improvements include:

  • A nearly 7% increase in patients with controlled diabetes and hypertension.
  • Over 222,000 more pediatric patients screened for weight assessment and nutrition counseling.
  • Over 621,000 more patients screened for body mass index (BMI) screening and follow-up in adults.

To recognize these achievements, HRSA awarded Community Health Quality Recognition (CHQR) badges to nearly 1,000 health centers across the country for excellence in clinical quality. Of these, HRSA recognized nearly 400 Health Center Quality Leaders (HCQL) for improving health outcomes and providing high-quality care for their patients, awarding the top 30% of health centers with a Gold, Silver, or Bronze badge:

  • The top 136 health centers were awarded the Gold HCQL badge.
  • 147 and 109 health centers earned a Silver or Bronze HCQL badge, respectively.
  • 640 health centers received a National Quality Leader (NQL) badge for meeting or exceeding national benchmarks in at least one clinical quality area, including cancer screening, behavioral health, diabetes health, and heart health.

Additionally, HRSA awarded two new badges to 242 health centers, representing 18% of health centers. The preventive health badge recognizes clinical quality in primary preventive health, and the high-value care badge recognizes improvements in clinical quality while minimizing costs.

HRSA also announced that it awarded over $53 million to 52 Health Center Controlled Networks (HCCNs). The newly awarded HCCNs will support over 1,300 health centers nationwide to increase their capacity to improve quality of care, support population health, and engage in value-based care delivery.
