    Diabetes affects over 38 million Americans, disproportionately impacting underserved communities where access to consistent, quality care can be a challenge. Effective diabetes management and prevention are critical to closing care gaps, and data-driven insights are reshaping how healthcare professionals identify, track, and manage at-risk populations. In observance of National Diabetes Month, here are five powerful ways data-driven insights are transforming diabetes care by enabling more proactive, efficient and community-centered approaches.

    Proactively Identifying At-Risk Populations

    Data-driven insights empower care teams to identify, track and prioritize individuals most at risk for developing diabetes or experiencing complications. By analyzing clinical data, patient history and social drivers of health (SDOH), these insights can stratify populations, helping care teams tailor interventions, screenings and programs to those who would benefit most.

    Why It Matters: Nationwide, over 38 million people have diabetes but nearly 98 million have prediabetes. Early identification allows for timely intervention, slowing disease progression and preventing costly complications.     A1c testing is a key measure of blood sugar control, essential for managing diabetes and identifying prediabetes.
