Direct Contracting Explained: How Northwell Direct Cut Employer Costs 20%
On the 9/26 episode Megan welcomes Nick Stefanizzi, CEO of Northwell Direct and they discuss how every health system that considers selling directly to employers hits the same wall: The discounted rate lands on patients you may already have, and nobody can prove the trade works. Nick Stefanizzi says it does. He is the CEO of Northwell Direct, Northwell Health’s for-profit direct-to-employer company, where an employer or union agrees on price with the health system and no carrier sits in the middle. In December, it signed the 170,000-member 32BJ Health Fund, the largest direct contract of its kind in the country, with the fund projecting 20% savings in year one. Stefanizzi told HFMA the business contributes more net-new volume and margin than it cannibalizes, and is profitable on its own P&L.

Healthcare Isn’t Broken – It’s Designed to Keep You Confused
On the 9/27 episode Dr. Craig welcomes Adam Russo, Esq., CEO and Co-Founder of The Phia Group to discuss how the healthcare system is intentionally opaque, designed to keep us confused and compliant rather than informed and empowered. 98% of procedures are planned, yet we spend more time researching TVs than questioning doctors or prescriptions, driven by fear and conditioned acceptance. Vertical integration across the supply chain (hospital systems owning physician groups, PBMs, and pharmacies) creates closed loops that drive prices up without antitrust scrutiny. Companies that engage employees openly see better retention, health outcomes, and productivity. Ask questions about generics, billing accuracy, and out-of-network costs, and demand to see how your healthcare dollars are spent.

The conversation centers on how healthcare systems need to move beyond simply archiving data to implementing proper governance policies that address retention periods, approval workflows, and the balance between legal obligations and clinical continuity of care. McGuire highlighted the importance of distinguishing between data with clinical, research, and legal value, and addressed how acquisitions present opportunities to establish proper governance structures rather than inheriting unmanaged data assets. The discussion also touched upon the role of AI in data lifecycle management, noting that well-governed data provides better foundation for AI initiatives, and concluded with advice for CIOs to begin with data inventory and pilot programs in the archive system.
